A secondment opportunity has arisen for an experienced clinician to support this developing service to make a difference in supporting successful discharges for patients requiring 24hr care home placement.
Applicants require experience in the complexities of our growing number of patients from adult and older adults services requiring 24hr care home placement to meet their complex physical and mental health needs.
A key function of this role will be to support discharge planning from inpatients wards, care home assessments and the transition to care homes for up to 6 weeks post discharge. The post holder will also take a lead in supporting care home staff with education around supporting patients with mental health needs and complex co morbidities.
Main duties of the job
The post holder will work alongside adult and older adult inpatients services as well as the Care Home Support Service to improve the quality of life of care home residents, reduce hospital admissions and facilitate early discharge from hospital.
You will:
- Lead the service for adults who are inpatient in an inpatient setting who meet the criteria for permanent placement into a care home and have a dementia and/or a functional mental illness.
- Focus on supporting discharge from acute mental health wards to a care home setting, in particular supporting the transition from hospital setting to placement and working alongside care home staff to settle new residents.
- Manage mental health practitioners.
- Influence the service delivery ensuring clinical excellence supported by evidence-based practice.
- Provide specialist advice, support and consultancy to health professionals, patients, and relatives.

- Appropriate degree or equivalent HCPC/NMC mental health registration

Skills and Knowledge
Essential criteria
- • Experience of working in dementia care and with patients with severe and enduring mental illness
- • Substantial experience of mental health care and working across different settings
- • Experience of working with patients with multiple comorbidities and physical frailty
- • To have a good knowledge of community services both generic and mental health
Desirable criteria
- • Previous Community Mental Health Services experience
- • Experience of working in or alongside care homes
