A unique opportunity to shape the future of community care by strengthening the mental health input from our mental health practitioners within the Community Therapy Service (CTS) . This 12-month secondment has been developed as a direct outcome of a Quality Improvement programme, with a clear focus on enhancing the existing mental health input as part of the multidisciplinary community team. The successful candidate will play a pivotal role in leading and developing this area of practice, supporting the service to evidence the need for sustainable Mental Health senior clinical leadership and having a clearly defined role for Mental Health within CTS. If you are passionate about combining physical and mental health, service development, and working collaboratively across system partners, this role offers the chance to influence real change.
We are looking for:
- Registered health professional OT or Physio
- Significant experience working with adults with mental health needs
- Experience of working within community or integrated care settings
- Ability to work autonomously and influence across multidisciplinary teams
- Strong communication, leadership, and relationship-building skills
- Experience or interest in quality improvement or service development

Main duties of the job

- Lead on the development and clarity of mental health input within Community Therapy Services
- Provide expert clinical advice and support to multidisciplinary team managing patients with physical and mental health needs
- Work collaboratively with AHPs, nursing teams, ops and clinical leads at all levels to provide high quality patient care
- Identify gaps in provision and develop pathways, guidance, and training to support staff
- Contribute to quality improvement activity, including data collection, evaluation, and reporting of outcomes
- Act as a clinical role model, promoting best practice in mental health care within community settings
