The Oxfordshire Community Diabetes Service is a well established team of Diabetes Specialist Nurses committed to delivering high quality, person-centred care across the county.

We are seeking an enthusiastic and motivated nurse to join our friendly and supportive team. This is an opportunity for someone who is eager to develop their nursing career in diabetes and is passionate about making a difference to the lives of people living with diabetes.

Main duties of the job

A successful applicant would participate in the following activities:

  • To provide excellent care for patients with diabetes across Oxfordshire.
  • Deliver various diabetes education programmes to Health Professionals in Oxfordshire.
  • Deliver countywide structured education programmes for people with Type 2 diabetes.
  • Provide telephone and email advice on diabetes management to Health Professionals in Oxfordshire.

Detailed job description and main responsibilities

Subject to funding, support may be available to undertake a university-accredited diabetes course and, in the future, a Non-Medical Prescribing qualification.

Following completion of the required training, the postholder will:

  • Must have access to a car and be able to travel across Oxfordshire to deliver services.
  • Funding permitting, opportunities may be available to complete a diabetes qualification and, subsequently, a Non-Medical Prescribing course.
  • Provide evidence-based diabetes care for adults with Type 1 and Type 2 diabetes, working autonomously to assess, manage and review patient care.
  • Promote self-management, deliver patient education, and work collaboratively with multidisciplinary teams to achieve the best outcomes.
  • Request and interpret diagnostic tests, manage diabetes treatments in line with local and national guidance, make referrals where required, and maintain accurate clinical records.

Applicants must have a full valid driving licence and have access to a vehicle for work. Adjustments can be considered in line with the Equality Act 2010 where required.

Essential criteria

  • NMC registration

Desirable criteria

  • To be an Independent Prescriber or willingness to undertake training.
  • Leadership qualification.

Skills

Essential criteria

  • Experience of working in a multi disciplinary team

Desirable criteria

  • Evidence of an ability to analyse and implement research and audit processes.

Contractual

Essential criteria

  • To have a driving licence and a car for work use, willing to travel across the whole of Oxfordshire.

Experience

Essential criteria

  • Previous experience of working with patients who have diabetes.

Desirable criteria

  • Demonstrate a willingness to try new things and expand learning and experience.
